What to do with the form
Once you've completed your form, we'll need the form and supporting documents.
You can bring it into a service centre.
If you can’t bring your form into a service centre, call us to find out how to get the forms to us.
Bring your ID into a service centre (if you’re a new client)
You need to complete this step if you’ve never received a benefit or ongoing payment from us. You need to bring your original government-issued identity (ID) document into a service centre so we can check it. You don't need an appointment to do this. If you have a partner, they need to do this as well. Checking ID in person helps us make sure we give support to the right person and protect your identity.
Examples of ID:
- passport
- driver licence or
- birth certificate.
If you can’t come in, or you don’t have government-issued ID, talk to us about ways we can help, call us.
If you have an agent acting on your behalf, they need to bring their own government-issued ID as well as yours.
