This page holds research consent information for clients to read before agreeing to take part in the research being conducted by Eleven on behalf of MSD (Work and Income).
Consent to take part in research
I understand:

Taking part in this interview is my choice and I can stop the interview at any time, for any reason.

Anything I say or if I decide to stop the interview won't affect any benefits I may get.

Any interview notes and information about what I say will only be available to Eleven and won't include anything that identifies me. All information about me will be stored safely.

The Ministry will only get research results in a summary report. It won't include anything that identifies me.

Eleven will destroy any private information they hold about me or what I said shortly after their report is given to the Ministry. All information will be destroyed by 30 June 2019.
