Your information if you ask for self-isolation help

This information applies only to the online forms you use to:

• ask for community support during self-isolation
• apply for financial help during self-isolation from the Ministry of Social Development (MSD).

Collecting and using your information

MSD collects your information to:

• identify and contact you
• work out how we can help you while you're self-isolating.

There's certain information we need to give you a benefit or payment through Work and Income. You can choose not to give us this information, but this could mean we can't help you. Your information will be managed in line with MSD's Privacy, Retention and Disposal Policies. It will be disposed of securely and appropriately once it is no longer needed.

Sharing your information

Your information could be shared with organisations outside of MSD in the following ways:

• If you've been referred to MSD through Ministry of Health, we may contact them to confirm that we're helping you.
• If you give us permission to share your information, we may refer you directly to a community organisation you've told us you want to work with.
