Proof of costs
Before granting Disability Allowance the following must be verified by a registered medical practitioner:
- that the person has a disability that is likely to continue for not less than 6 months
- that the person requires ongoing support
- that the person requires ongoing supervision or treatment by a registered health professional
The client must provide receipts as evidence of their extra costs. If a receipt is not available, a quote or estimate of the costs must be provided. The quote or estimate must be completed by an appropriate provider of such goods or services.
Give the client a ‘record of expenses’ sheet or card to help them keep a record of their expenses for the next review.
When treatment is provided the following must be verified by the registered health professional who administers the treatment:
- that the additional costs claimed are the direct result of the person’s disability
- that the costs are ongoing
- that the goods or services are needed and are of therapeutic value to the person
A Disability Certificate must be completed by the registered medical practitioner verifying the person’s disability and need for treatment, and a registered health professional must verify the ongoing treatment and the costs.
