Community Services Card

You could save on health-related costs with this card

A Community Services Card saves you money on:

  • prescription fees, if you don’t belong to a PHO 
  • fees for after hours doctor visits
  • GP fees for visits outside your PHO 
  • spectacles for children under sixteen
  • emergency dental care provided by hospitals and approved dental contractors*
  • travel and accommodation for treatment at a public hospital outside your area (special conditions apply, ring 0800 999 999 to find out more)
  • home help.

*Ask the dental provider if they are an approved contractor.

You may be able to get a card even if you work – simply check if your income is under the limit.

  • call us free on 0800 999 999 Monday to Friday 8am to 5pm, or
  • pick up an application form from your nearest Work and Income Service Centre or doctor, or

For families of 7 or more - 1 April 2012, the income limits increase by $7,703 for each additional child.
