Community Services Card
You could save on health-related costs with this card
A Community Services Card saves you money on:
- prescription fees, if you don’t belong to a PHO
- fees for after hours doctor visits
- GP fees for visits outside your PHO
- spectacles for children under sixteen
- emergency dental care provided by hospitals and approved dental contractors*
- travel and accommodation for treatment at a public hospital outside your area (special conditions apply, ring 0800 999 999 to find out more)
- home help.
*Ask the dental provider if they are an approved contractor.
You may be able to get a card even if you work – simply check if your income is under the limit.
If you think you qualify for a Community Services Card:
- call us free on 0800 999 999 Monday to Friday 8am to 5pm, or
- pick up an application form from your nearest Work and Income Service Centre or doctor, or
