Work and Income relies on medical practitioners to indicate a child or young person’s health condition and/or disability and to give information about their need for constant care and attention.
This booklet provides a step-by-step guide to completing the Child Disability Allowance medical certificate and gives examples of the type of disability that is likely to qualify the child or young person for the payment.
It has been written to help you understand why questions have been asked in the certificate, and how your responses are likely to be interpreted by Work and Income case managers.
