Mainstream Employment Programme: Forms
This checklist is used by Work and Income staff to work out if an individual with a disability is eligible for the Mainstream Employment Programme. Individuals and their placement specialists can use it as a guideline to make sure they have the right information with them when they talk to Work and Income staff.
Complete and sign the Privacy Statement form when you apply for the Mainstream Employment Programme.
Complete the Job Proposal if you are an employer and have a job opportunity for a Mainstream participant.
Complete the CODE application form if you are a Mainstream participant and you need extra adaptive equipment or services in your workplace. The form should be completed in conjunction with your supervisor and placement specialist.
Complete the training fund application form if you are a Mainstream participant, or the supervisor of a Mainstream participant, and you want to apply for Mainstream training assistance to do some study.
This form is used by Work and Income staff to check on progress and help the Mainstream participant to have a successful placement. Mainstream participants, supervisors and placement specialists can use it as a guideline for the process.
